Secure an affidavit of loss and go to the social security office.

try typing in your local social security office, they have a site for lost cards. if you can't do this, then look in your local phone book under Goverment office, and call and they will tell you what to do

If she is not sure that she lost it in the house the best thing for her to do is to go to her nearest Social Security and explain to them that she lost her card and she would like a new number. They will ask her questions and ask for copies of certain identification such as Drivers License, Birth Certificate, and any other information she may have in her name. It may take a month or more before she receives her card, but let them know that you have to get a new number and then call any bank, doctors or bills that she has in her name and change her S.S. once she gets her card. Then put an alert on her credit report so they can watch just in case anyone uses her old number and put bills in her name and let them also know that she has a new S.S. number so it can be updated.
